Fill out these 3 simple forms and email them back to [email protected]. along with the following:

Copy of your Insurance License.

Copy of your E&O Insurance Certificate - click here to buy E&O Insurance

Copy of a Voided Check for direct deposit

Copy of your Annuity Training certificate (if required by your resident state) - click here for training website

Copy of AML (Anti-Money Laundering) certificate. AML is required to be renewed every 2 years - click here for the LIMRA website, please submit a screenshot copy of the course and date that the training had been completed within the last 2 years

Corporate Licensing Procedures:

If you would like to be contracted as a corporation, or if you would like your commissions to be paid to a corporation, please send us the following in addition to the above requirements:

Copy of the Corporation’s Insurance License

Copy of the Corporation’s E&O Insurance Certificate

Corporation’s Tax ID #

Copy of either the Articles of Incorporation, meeting minutes, or a list of corporation principles
